Out-of-network provider

Out-of-network provider reimbursement is eligible with a flexible spending account (FSA), health savings account (HSA), health reimbursement arrangement (HRA), or limited purpose flexible spending account (LPFA- dental, vision, and preventive care only). Out-of-network provider reimbursement is ineligible with dependent care flexible spending accounts (DCFSA).
